We are pleased to invite you to attend our upcoming conference in Zagreb, where will be presenting the final results from our Erasmus+ project Education and Empowerment for Transition to Community.

The goal of the project was to support key competences of people with mental health issues before the discharge from psychiatric hospitals (wards) or long-term residential facilities with the aim to support their recovery.
The project has been a collaborative effort of Centre for Mental Health Care Development fromPrague (Czech Republic), Association for Psychosocial Assistance Susret (Zagreb, Croatia) and Wellbeing and Recovery College (Tartu, Estonia).
Participation in the conference is free of charge.
